PCOS fertility questions, answered

Real answers for getting and staying pregnant with PCOS

The questions women with PCOS ask me most, answered straight. I'm Tianna Trinidad, RN, a PCOS Hormone & Ovulation Specialist with 15+ years in high-risk Labor & Delivery and the NICU, and I have PCOS too.

Who is Tianna Trinidad, RN?

I'm a Registered Nurse (RN, MHL) with 15+ years of high-risk clinical experience in Labor & Delivery and the NICU, and the founder of Love Served Warm. I'm a PCOS Hormone & Ovulation Specialist, host of the PMOS & Prosecco podcast, and a woman with PCOS who has lived the long cycles, the confusing tests and the loss. I work online with women with PCOS who are trying to get pregnant, especially women of color who have been dismissed for too long.

What does a PCOS fertility coach actually do?

I don't hand you another list of things to try. I look at your cycles, your ovulation tests, your temperatures, your labs and how your days really run, and we find where your cycle is breaking down. Then we change a few things, not seventeen, and watch what your body does next. You get weekly time with me and support in between, so you're not guessing alone at 2 AM. I work alongside your doctor, never instead of them.

Can you get pregnant with PCOS if you don't ovulate regularly?

Yes. Many women with PCOS get pregnant, and irregular ovulation is one of the most common reasons it takes longer, not a dead end. The first step is knowing whether, when and how well you ovulate, cycle by cycle, instead of assuming. Once we know where your cycle stalls, we know what to support first.

I get a period every month. Doesn't that mean I'm ovulating?

Not necessarily. With PCOS you can bleed without releasing an egg. A period tells you a cycle ended. It doesn't tell you an egg was released, or that the second half of your cycle was long and strong enough to hold a pregnancy. That's why I look at the whole cycle, not just the bleed.

Why do my ovulation tests (OPKs) look almost positive for days, or positive more than once?

With PCOS, LH can run high, so strips can look dark for days, and your body can gear up to ovulate, fall short, then try again. Mucus can show up more than once too. A positive test isn't proof you ovulated. Pairing your strips with your temperatures and the length of the second half of your cycle tells us which surge actually counted.

I'm on Metformin, inositol, Letrozole or Clomid. Why am I still not pregnant?

The name on the bottle tells me what you're taking. It doesn't tell me what your body did. Medication can help you get to ovulation, but pregnancy also needs good timing and a second half of the cycle that holds. I help you see what your recent cycles were trying to tell you and what to ask your doctor before the next round. I never tell you to stop anything your provider prescribed.

What is Build, Surge, Hold?

It's how I read every PCOS cycle. Build: is your body building toward ovulation, or stalling? Surge: is the LH surge real, and does ovulation follow it? Hold: is progesterone strong enough, and long enough, to hold a pregnancy? The order matters. If Build is the problem, fixing Hold won't help, and if Hold is the problem, more ovulation help won't fix it.
